Senior Case Management
To enhance the functional independence and economic and social well-being of homebound functionally- and/or cognitively-impaired adults age 60 and over who reside in Forest Hills and Rego Park, Queens Community House’s Senior Case Management program provides assistance with the process of planning, arranging, and coordinating services and resources for them.  We also provide information and referral to services to homebound elderly throughout Queens.  

Program Hours: Case managers are available to provide assistance
Monday through Friday from 9:00 a.m. to 5:00 pm.

Services offered to eligible elderly through case management department are:
  • Information and referral
  • Home-delivered meals, kosher and non-kosher
  • Assistance with individual benefits and entitlements
  • Advocacy on behalf of the senior for resources, services, and opportunities
  • Expanded In-home Services for Elderly Programs (EISEP); Homemaker Personal Care (HMPC); and Housekeeping/Chore (HSCH). These services are available at a sliding scale cost from a minimum of 4 hours weekly to a maximum of 20 hours weekly
  • Shopping Assistance Chore - once a week (see Intergenerational Program)
  • Transportation service to doctors (see Transportation services)
  • Telephone Reassurance (see services provided under Rego Park Senior Center and Pomonok Senior Center)
  • Social Adult Day Services (see separate program listing)
  • Referral of ineligible clients to the appropriate non-DFTA (Department For The Aging) system
  • Post-referral follow-up to confirm provision of services
We determine an elderly person’s initial and ongoing eligibility for these services by working with the individual and their family, using various methods of assessment, including:
  • Identification of needs and capacities through a comprehensive intake and assessment process, including an in-home assessment.
  • Determination of eligibility for Department for the Aging (DFTA)-funded in-home and social adult day services
  • Development of a comprehensive and coordinated care plan including services and/or other interventions to meet participant needs and goals.  
  • Monitoring of the care plan, including but not limited to authorization of, or referral to, services.
  • Reassessment of the client, and updating of the care plan in light of findings.
